Requesting Case-specific Information
Use this form to request more information about a specific child. We'll put you in touch with the child's family, local doctors who have reviewed the child, or provide case-specific medical information as neccessary. In instances where the family prefers privacy, we may not be in a position to provide medical information. However, all people who have supported a child will be able to contact the relevant family.
